Abstract

Cystoid macular edema after cataract extraction with implantation of an intraocular lens is considered to be the most frequent and troublesome complication. The advantage of topical indomethacin had been previously proven, but the influence of oral administration had not been investigated. The purpose of this study is to Investigate the effect of the using of indomethacin on the incidence of cystoid macular edema after cataract surgery with an intraocular lens implantation. Two groups of patients with matured and immatured senile cataract undergone extracapsular cataract extraction with implantation of an intra ocular lens had been reported. Each group consisted of 40 patients. Indomethacin group received 25 mg indomethacin orally 2 hours preoperativelly, as well as at 2, 6, and 10 hours postoperativelly then continued for 5 days postoperativelly. Ocular examinations were performed on postoperative days 1, 3, 7, 14, 28, 60, and 90. No Incidence of cystoid macular edema in group indomethacin was found and in placebo group was 2,5% (x2 = 0,001; p> 0,05). After seven days observation on the effect of indomethacin to the postoperative inflammatory reaction demonstrated that there was insignificantly difference between indomethacin and placebo group (x2 = 2,15; p >0,05). Observation on the therapeutic side effect phenomenon indicated that Indomethacin was relativelly safe even if used in the aged patients.Key Words : oral indomethacin - cystoid macular edema - cataract operation - intra-ocular lens -inflammatory reaction

Abstract

Infertility is a problem among child bearing age couples. TORCH infection as one of causative factors infertility must be investigated of its role. The objective of this study is to compare TORCH antibodies seropositivity of infertile cases to pregnant women. Sera from 67 infertile women and 21 pregnant women were used for the study. The results are as follows: the mean of IgG anti-CMV and IgG anti-HSV2 titre of Infertile group are significantly higher than titre of pregnant group. The individual who has been exposed to HSV-2 will have a higher risk to Infertility than who has not. Coincidence infections of TORCH are similar between two groups. It can be concluded that problems of TORCH in infertility must be inferred to any other related factors.Key Word: TORCH - infertility - seroimmunological test - IgG anti-CMV - IgG anti-HSV2

Abstract

The severe affected thalassemla syndrome Is common In Southeast Asian countries. Considering that the feature of its peripheral blood smear Is typical, this study was aimed at investigating the diagnostic value of the blood smear in thalassemia syndrome. Sixty five patients with severe anemia and splenomegaly who were admitted or came for follow up to Sardjito General Hospital in the period of 1996 and 1997 had been enrolled In this study. The peripheral blood smear was assessed, whether thalassemla or nonthalassemia, by two laboratory technicians, each of them read blindly each smear at two occasions with at least two weeks interval. The gold standard for diagnosis of thalassemla major was the evidence of increased HbA2 (>3,5% of total Hb) In both parents and for thalassemla-hemoglobin E disease was the increased HbA2 in one of the parents and the presence of HbE in the other. The measurements of HbA2 and HbE were carried out by quantification of HbA2 fraction following hemoglobin electrophoresis on cellulose acetate membrane (CAM) electrophoresis. The results of blood smear reading showed good intrarater agreement with kappa = 0.691 by the first rater and 0.634 by the second rater). The interrater agreement was high moderate to good (kappa = 0.567 - 0.728). The first reading by the first rater, the second reading by the first rater, the first reading by the second rater, and the second reading by the second rater showed sensitivities of 0.780, 0.780, 0.878, and 0.805 respectively; specificities of 0.708, 0.958, 0.542 and 0.792 respectively; positive predictive values of 0.820, 0.969, 0.766, and 0.868 respectively; and negative predictive values of 0.650, 0.719, 0.722, and 0.704 respectively. The peripheral blood smear has high sensitivity and specificity for diagnostic test of thalassemia syndrome.Key words: thalassemia syndrome - blood smear - hemoglobin electrophoresis - sensitivity and specificity -predictive value

Abstract

Glaucoma is an Important cause of blindness, in particular neovascular glaucoma which is a complex condition and always becomes challenging for the clinician in the past, present and the future. A 54-year-old man suffering from blindness of neovascular glaucoma following central retinal vein occlusion in the right eye with no response to Pan Retinal Photocoagulation (PRP) and open angle glaucoma in the left eye has been reported. Systemic conditions as a predisposing factor were not found, so the local ocular background i.e open angle glaucoma was suspected to be the cause of central retinal vein occlusion. In this case the management must be focused on the left eye to avoid further complication. The right eye was on end stage and no response to PRP and the visual acuity Is no light perception (NLP). Enucleation should be considered as the high intraocular pressure (IOP) and ocular pain persisted.Key words: neovascular glaucoma - central retinal vein occlusion - open angle glaucoma - Pan retinal photocoagulation

Abstract

Liver cytochrome P450 microsomal protein metabolizes certain xenobiotic compounds into reactive intermediates which covalently bind to DNA, RNA or protein of nucleus. The action of cytochrome P450 can be Influenced by nutrition status and environmental pollution. Aflatoxin Bi (AFB) is the environmental exposure problem in the countries with tropical climate. Aflatoxin Ell can also be oxidized by cytochrome P450 to DNA and protein - binding metabolites and can give rise to cell injury, mutation and cancer. The objective of this study is to Investigate the effect of dietary vitamin A (deficient, adequate, fourfold-adequate) against the toxic effect of AFBi in the liver cytochrome P450 microsomal protein of rats. Variance analysis of the result showed that the interaction of the vitamin A in the diet and treatment with AFB, had no significant effect. However the rats with dietary deficient and adequate in vitamin A, treated with AFBi showed significantly increased the cytochrome P450 (p< 0.001 I. The dietary fourfold-adequate in vitamin A was able to maintain the concentration of cytochrome P450 in liver microsomal protein of rats treated with AFB1 within normal range. It Is concluded that the effect of dietary vitamin A against the toxic effect of AFB, in liver cytochrome P450 microsomal protein of rats depends on the vitamin A concentrations in the diet consumed.Keywords: dietary vitamin A - aflatoxin Bt - liver microsomal protein - cytochrome P450 - Rattus norvegicus

Abstract

Nonperiodic form of B. malayi was recently discovered as a new infraspecific variant (new subspecies) of filarial parasite found among Dayak Indigenous people living scattered in deep forest of East Kalimantan. In this research, clinical or blood (including membrane filter concentration method) examinations were carried out on Bugis or Banjar tribais in kampongs or more open areas in East Kalimantan. The malayan filariasis infections were detected among them, but not in high prevalence nor microfilarial density, such as found in two villages in Mahakam delta or another one In Bontang area, Kutai regency. The clinical manifestations, however, were not severe nor specific.Key words: Brugia malayi - nonperiodic form - Bugis and Banjar settler - natural selection pressure - parasite dispersal.

Abstract

Mercury is one of the potent allergens which can induce contact hypersensitivity. This substance however, is commonly used for cosmetics and medical purposes. The exact mechanisms underlying the mercury-induced hypersensitivity remain unclear. Studies on mercury contact hypersensitivity in human were hampered due to ethical reason. This investigation was, therefore, aimed at studying hypersensitivity reactions In animal model. Thirty five female Wistar rats, aged three months were divided into three groups of treated animals and two groups of controls. The treated groups were sensitized with 2,5%, 5%, and 10% HgCl2 In petrolatum for 10 days. At day 14, animals were challenged with 5% HgCl2 in 97% ethanol. Contact hypersensitivity was assessed by measuring the ear swelling before and after ear challenge. Specificity test was carried out by challenging the sensitized animals with K2Cr207. The results showed that the, peak levels of ear swelling could be achieved by applying 10% HgCl2 as compared to other groups of animals. Chromium (K2Cr207) ear challenge following 10% HgC12 sensitization failed to induce ear swelling. It can be concluded that the mercury hypersensitivity reaction in this animal model is antigen-specific.Key words: allergy - contact hypersensitivity-mercury - DTH reaction - Wistar rats

Abstract

DNA sequencing of the 0-globin gene was done and clarify the Hemoglobin M-Saskatoon at molecular level.A boy was detected to suffer from 13-thalassemia since three year old. At four years of age, he underwent splenectomy due to severe splenomegaly. At 10 years of age blood sample was withdrawn and enzymatic sequencing of blood lympocyte DNA showed a mutation at Codon 63 (CAT->TAT). Therefore, hemoglobin M-Saskatoon was diagnosed. This mutation was also found in his mother detected by using NLA Ill restriction enzyme which digests the wild type of DNA at the CATG/region. This is the first report to demonstrate sequencing technique identifying hemoglobin M instead of using the biophysical examination of the blood oxygen binding affinity.Key words: /3-thalassemia - hemoglobin M - Saskatoon - DNA sequencing - endonuclease restriction enzyme
